The personal information you supply YDAP in this form will be processed in accordance with their own General Data Protection Regulation (GDPR) and the Data Protection Act 2018 policies and procedures.
YDAP may store and share some relevant personal details such as name, address, age, gender, contact number and any health information that is needed to deliver their services, as well as sharing anonymous data for monitoring purposes. YDAP will ask you for explicit consent if they want to share non anonymised details for the above reasons (for example to provide a case study).
The information you provide in this form will also be held within the Links Carepath system which supports YDAP services. Your form, and the information you have provided, will only be sent to YDAP.
The YDAP Team are responsible for day-to-day management of the Links Carepath system – the Team is based within Together for Children, and team members observe the Together for Children’s privacy and confidentiality standards. Anonymised data may be shared with other Together for Children departments, City of Sunderland Council and the Office for Health and Disparities to monitor and improve the service, or for the prevention or detection of crime and fraud where the law allows this.
